In a large skillet, heat 2 tablespoons of olive oil over medium heat. Add the sliced onions and cook, stirring occasionally, until they are softened and caramelized, about 20-25 minutes. Season with salt and pepper. Add balsamic vinegar (if using) during the last 5 minutes of cooking.
While the onions are caramelizing, cook the sausage in a separate skillet over medium heat, breaking it up with a spoon, until fully cooked and browned. Drain any excess grease.
On a lightly floured surface, stretch or roll out the pizza dough to your desired thickness and shape.
Transfer the dough to a pizza peel dusted with flour or cornmeal, or directly onto a baking sheet.
Spread the caramelized onions evenly over the pizza dough. Top with the cooked sausage, then sprinkle generously with shredded mozzarella cheese.
Drizzle the pizza with 1 tablespoon of olive oil.
Carefully slide the pizza onto the preheated pizza stone (if using) or place the baking sheet in the oven.
Bake for 12-15 minutes, or until the crust is golden brown and the cheese is melted and bubbly.
Remove the pizza from the oven and let it cool slightly before slicing and serving.
Garnish with fresh basil (optional).
